Can we have a form like this https://cocbt.com/contact/ that then sends an email us at bhidc@utah.edu? I’d just like to add a non required phone number, a box where they check it’s ok to leave a voicemail at that number, and preferred contact method check box for phone or email. We don’t need preferred provider and the private pay/insurance fields.
